- Identify who the Duly Authorized representative of the private provider is based on F.S. 553.791 (1f).
- A copy of the Pinellas County Property Appraiser’s printout listing who the fee owner of the land is that is authorized to sign the NTBO form and hire a private provider. If the fee owner of the land is a corporation, a copy from Florida Sunbiz showing the officer(s) registered agent of the corporation who can sign the NTBO and authorize the use of a private provider.
- A signed, notarized, and completed Notice to Building Official (NTBO) form for the fee owner’s decision to use a private provider based on F.S. 553.791 (4c).
- A copy of all insurance certificates (COI). These will need to list the project description at the bottom of the page under the “Description of Operations” area, including the address of the project. Also, the City of Seminole needs to be listed as a certificate holder on the insurance forms. The City of Seminole address is 9199 113th Street, Seminole, FL 33772.
- A qualification statement and Florida State Standard Inspector license numbers of all of the private provider plan reviewers and inspectors based on F.S. 468.
- Completed permit application with a full set of architecturally or engineered signed / sealed approved plans.
- Contractor’s Authorization Letter (if contractor has been assigned).
- Recorded Notice of Commencement
